About telehealth visits
Telehealth uses electronic communications to enable a healthcare provider at one location to diagnose, treat, or counsel a patient at another location.
You may need to use video, audio, secure messaging, or store-and-forward technology depending on what your visit requires.
You have the right to a comparable in-person appointment instead. Decline this consent to receive only in-person care.
Risks
Technical failure may interrupt your visit. We will reschedule promptly when this happens.
Although we use end-to-end encryption, no transmission is perfectly secure.
Without a physical exam, some conditions cannot be fully evaluated by telehealth and may require an in-person visit.
Your rights
You may withdraw this consent at any time without penalty by visiting Settings → Consents.
Any recordings made of your telehealth visit are part of your medical record.
You may request copies of any documentation associated with the visit.
